Soft tissue fillers are injected under the skin for instant wrinkle release. Just like in leather, wrinkles are caused by a breakdown in collagen and elastin underneath the skin. These gel-like implants are injected underneath the skin and act as bolsters to prop up or pop up the furrow. Popular sites for injection are the smile folds that go from the corners of the nose to the corners of the mouth, lip wrinkles, scowl furrows between the brows, and acne scars. These can be injected during a lunch hour. Results are immediate, and the small injection sites can be immediately covered with makeup.
Collagen was available for years and typically lasts 3 months. It is a good way to “try on” a filler and see if they are for you. It is available as Cosmoderm® and Cosmoplast®. It comes premixed with lidocaine, a numbing agent.

The disadvantage of collagen was the short duration. Now out are longer lasting fillers, such as Restylane® and Radiesse®.
Hyaluronic acid, a gel that is naturally occurring throughout the body tissues. It is part of what gives baby cheeks their pinchable smoothness. It is available as RESTYLANE®, made by Q-Med AB, a Swedish company, and has been in wide use in Europe since 1996. Its advantages are its hypoallergenicity (no allergy test is needed) and durability; it typically lasts twice as long as collagen. You can feel it under the skin but it doesn’t appear bumpy. It can cause bruising. These gradually break down into water, and may stimulate new collagen to form.

Even longer lasting is Radiance, which is calcium hydroxyapatite, the same component that provides structure in the bone. Injected underneath the skin, it stays soft and lasts 2+ years. It is an opaque gel and comes in pre-filled syringes. Its main advantage is its long duration. This requires a numbing injection prior to injection of the Radiance® under the skin.
The cost of these reflects their duration are listed below. Fillers tend to last longer in areas that don’t move (eg. acne scar correction lasts longer than lip wrinkle correction, because lips move) so Botox Cosmetic ® can help make fillers longer lasting by relaxing the muscle causing the wrinkle, such as around the lips or around the furrow between the brows.
